Frequently Asked Questions

What does Cashed based/ out-of-network physical therapy mean?

Cash-based physical therapy means that we are out of network with all insurance companies and do not directly bill through insurance (though you may be eligible for reimbursement directly from your insurance company).

Our patients pay us directly each visit for their 1-on-1, personalized physical therapy session. This allows us to focus all of our attention on the patient and perform treatments to focus on patient goals without being dictated by insurance rules and regulations.

Do I need a prescription from my doctor to come see you?

No prescription is needed to be seen. In Ohio, we have what is called direct access, meaning that you can see a Physical Therapist without a prescription. See Section 4755.481 of the Ohio Revised Code, Treatment without prescription or referral.

What insurances do you accept?

We are an out-of-network provider, meaning we do not take health insurance. You can certainly use your out of network insurance benefits.

You can also use your health savings account (HSA) or flex spending account (FSA).

While we do collect our fees directly from our clients, we provide an itemized superbill to submit to the insurance company.

By submitting your receipt, you may be able to recover a percentage of the cost you pay upfront (typically 60-80%) from your insurance company, depending on your "out of network" coverage.

Why Cash pay/ out-of-network PT vs. Traditional PT?

Insurance companies in the United States are continually cutting reimbursement rates for rehabilitation services and dictating how clinicians treat patients.

With this decline in reimbursement, many clinics are forced to treat multiple patients in the hour (this means that therapists are seeing 3+ patients at the same time) and lower patient care standards in order to make ends meet.

These clinics rotate patients in and use assistants and techs to complete treatments. Patients do not receive the individualized care they need, and are encouraged to go through exercises with little to no supervision.

With a cash based practice, we are able to  treat patients in a way that is actually best for THEM as we are not regulated by the insurance companies.

This allows us to provide the highest quality care for our patients, while keeping them 1- on -1 with their doctor of physical therapy for the entire visit, every visit.

This increase in quality results in our patients getting better, faster than traditional insurance based physical therapy.

How do I pay?

We accept cash, check, Venmo, Cash App, PayPal, and Health Savings Account, Flexible Spending Account.

How long does a session typically last?

Evaluations and follow-ups are one-on-one for an hour, in which you will get the full attention of your performance physical therapist for the entirety of the session.

How many sessions will I need?

Every case will differ based on your unique aspects. These include:

  • Severity of symptoms/injury
  • How long you have been experiencing your pain/injury
  • Stress
  • Emotional/mental wellbeing
  • Nutrition
  • Immune health

Following your assessment, your physical therapist will recommend a plan of care based on your needs.

How often will I have to attend?

We typically see our clients 1-3x/week in the beginning to help manage pain/symptoms and build a strong base. After a few weeks, once the pain subsides, we reduce the frequency. This ensures that we get your feeling and moving better than you were before, as well as help you to feel confident in being able to return to your activities.

What is your cancellation policy?

We require 24 hours notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours notice, or miss their appointment, will be charged a cancellation fee of $75. Patients that "No-Show" for their appointment will be charged 100% of the scheduled appointment cost. It is up the discretion of the therapist to cancel the appointment if arrival is later than 20 minutes of scheduled arrival and will be subject to cancellation fee of $75. Our policy is to be fair, not only to Banna Physical Therapy and Performance, LLC, but also to other patients on our roster who might like to be seen.

What is a superbill?

A superbill is a medical receipt that we will provide you with at the end of your session. If you have out-of-network benefits, you may submit this to your insurance for reimbursement based on your health insurance plan.

What if I want to keep seeing you after my plan of care ends?

We offer a continuity program where you will have access to your provider and ability to schedule a session on a monthly or bi-monthly plan.
